Well well...here we are talking about use of whistle in climbing.
If you pack one-it does not increase wt. of rucksack but use of this item is bit hanky panky.
In ungle it will attract wildlife-it does not scare them. Also it does not help in guessing of direction from where someone is blowing his lungs out !
It can only tell someone ( if someone is near to you ) that some human being is around and require help.
When in climbing-it is one of the most dangerous thing to use it. I do not know about other mountain ranges but in Himalayas ( I tihink mountain is mountain..so seems it suits to all mountain ranges ) you can use whistle as it can trigger rock fall OR avalanche.
Both are very very big factors of failure of expedition and getting someone killed if not badly hurt !
Rest of the list seems to be fine-I would add
1. Good Sleeping Bag ( Down)
2. Good tent
3. Some life saving medicines like Derriphyline,Asthalin etc etc.
4. Muscle relaxant ointments /sprays & First Aid Kit
5.
